Make a referral to Celestial Care

We accept referrals from local authorities, trusts and placement commissioning teams across England and Wales. Tell us about the young person and we will give you an honest, evidence-based matching decision within one working day.

The process

From first call to a settled placement

We keep the referral process short and transparent, because delay rarely serves a young person well — but neither does a placement that was never the right match.

    You complete the form below or call the placements team. We acknowledge every referral the same working day.

    Impact and matching assessment

    Our home manager reviews risk, needs and the suitability of a single-occupancy home before any offer is made.

  3. 03

    Planned introductions

    Where time allows we arrange a visit, an overnight stay and a written transition plan agreed with the young person.

    Days 2–7

  4. 04

    Placement and review

    A placement plan is agreed within 24 hours of arrival, with a 72-hour settling review and formal reviews every 28 days.

Understanding what has and has not worked before helps us plan a stable transition.

Include the number of previous placements and what caused them to end, if known.

Risks and needs

An honest picture of risk protects everyone — the young person, the existing group and our staff. We will never decline a referral for being candid.

Self-harm, going missing, exploitation, substance use, offending, risk to or from others, and any current risk management plans.

Emotional and mental health needs, diagnoses, physical health, communication needs, interests, hobbies and what the young person says they want.

Upload the current risk assessment, care plan, PEP, EHCP and any recent reports. Accepted formats: PDF, Word, JPG or PNG, up to 10MB per file.

Data protection and consent

Celestial Care Ltd is the data controller for the information you submit. We process it under UK GDPR Article 6(1)(f) and Article 9(2)(b) for the purpose of assessing a placement and safeguarding a child. Information is held on encrypted UK-based systems, accessible only to the matching panel, and securely deleted after 12 months where no placement proceeds. Full detail is set out in our privacy policy.

Questions we are asked most often

How quickly will you respond to a referral?
Every referral receives an acknowledgement the same working day and a matching decision within one working day. Emergency enquiries are triaged immediately by our on-call manager.
Do you accept emergency and same-day placements?
Yes, where we can do so safely. Nova House is a single-occupancy home, so we assess carefully whether we can meet the child's needs there before accepting any placement, and we will tell you honestly if we are not the right match.
What information do you need to assess a referral?
A current risk assessment, the most recent care plan or placement plan, any education, health and care documentation, and details of the young person's legal status. Documents can be uploaded securely with this form.
How is our data protected?
Referral information is processed under UK GDPR on the lawful basis of legitimate interests and the safeguarding of children. Data is stored on encrypted UK-based systems, restricted to the matching panel, and deleted after 12 months if no placement proceeds.
